چکیده

A recent study of the postextrasystolic T wave changes, associated with interpolated premahire ventricular beats, revealed that this phenomenon is not rare and, unlike the T wave changes which occasionally occur in the first sinus beat following the more common premature ventricular beats with compensatory pauses, are not usually related to the presence of significant heart disease.’ Despite the establishment of the clinical significance of this phenomenon, the cause has remained obscure. One theory was that of Scherf2 who proposed that the postextrasystolic T wave changes seen after interpolated extrasystoles are secondary to aberrancy in ventricular depolarization. Although several electrocardiograms of the patients described in a previous study demonstrated slight aberrancy in the QRS complex of the postextrasystolic beat, many of them did not, and in those that did, the T wave changes were in excess of what might be expected from secondary T wave changes of QRS aberrancy.1 An opportunity to further evaluate this aspect of the problem arose when a patient was observed to be having occasional interpolated extrasystoles with postextrasystolic T wave changes during a routine clinic visit.

Postextrasystolic T wave changes and angiographic coronary disease.

The significance of postextrasystolic T wave changes in beats following induced extrasystoles was assessed by angiography in 55 patients. These T wave changes were found in 81 per cent of coronary artery disease patients but also in 68 per cent of patients with normal coronary arteries (PNS). All patients with normal baseline electrocardiograms and normal coronary arteries showed postextrasysto...

Bigeminy in ventricular interpolation.

T he occurrence of a premature beat between two consecutive conducted sinus beats is termed an interpolated premature systole or extrasystole.1 An interpolated premature ventricular systole can effect the conduction of the subsequent sinus beat by concealed retrograde penetration into the His-Purkinje system or AV node or both. This retrograde concealed penetration can result in a prolongation ...

Evaluation of left ventricular residual function using postextrasystolic potentiation. Relation between systolic time intervals and angiographic study.

The effects of postextrasystolic potentiation (PESP) on systolic time intervals and left ventricular wall motion were studied during diagnostic cardiac catheterization in 20 patients (4 normal individuals, 11 patients with coronary artery disease and 5 patients with idiopathic dilated cardiomyopathy). Postextrasystolic changes in the aortic pressure and systolic time intervals were measured fro...

Postextrasystolic alterations in refractoriness of the His-Purkinje system and ventricular myocardium in man.

The changes in refractoriness of the His-Purkinje system (HPS) and ventricular myocardium (VM) that are associated with the occurrence of postextrasystolic beats in man are unknown. Accordingly, using a pacing model of the cycle length changes created by a ventricular extrasystole-postextrasystole sequence, we measured retrograde HPS and VM relative and effective refractory periods (RRP and ERP...

The effects of right atrial and ventricular pacing on the auscultatory findings in patients with mitral valve prolapse.

Fifteen patients with midsystolic clicks associated with mitral valve prolapse were studied in order to assess changed in ausculatatory findings produced by pacing-induced variations in cardiac rate, rhythm, and conduction. As the heart rate was increased in stepwise intervals to the maximum possible extent by right atrial pacing (RAP) in 14 patients, the interval between the Q wave and the cli...
